About Us
Our Placement Process
We schedule a personal visit at a location of your choice. This can be a video visit if preferred. At that time we get to know the individual and family members. In this process we complete a biopsychosocial assessment. We discover their daily activity needs, medication assessment and budget. With this information we complete an individualized plan which includes our observations, medical provider recommendations, family requests and most importantly, the individual's desires. From this we form a care plan and recommend a placement matching their needs, with safety and happiness being a top priority.
Once the above process is complete we schedule tours of potential, well-vetted communities. If a community is selected we work with the community and the individual to create a smooth transition. We then follow-up with the individual, their family and facility multiple times within the initial days, weeks to come, often longer.

Avalon becomes your personal and family advocate, providing personalized, professional and compassionate guidance to assisted living and senior care, at no charge. We clearly guide you through this process in a personal, "high touch" manner.
Avalon Referral Services was founded by accomplished and university trained social services professionals, health care professionals and occupational therapist professionals
We believe in evaluating the entire person and understanding their family's needs as well. This includes an extensive personal visit to get to know the senior. We want to understand their physical, emotional and spiritual needs.
Avalon Referral Services only works with well vetted, well run prospective residences, we would be absolutely comfortable recommending to our own loved ones. Sadly, many potential homes, do not make our cut.
How & Why We Started
When searching for an advocate to help us in finding assisted living for a loved one, we discovered the referral service process provided little to no service at all. They added to the confusion and we felt like another number in their sales process. That was no service at all and a "no-go" for us. We wanted someone who would get to know and understand our loved one's needs. This means understanding their desires, their physical, spiritual and emotional well being.
We knew we could do better.
Advocacy Services
Advocacy Services
From time to time it becomes apparent a residential community is not the best, or preferred solution for the individual or family. In these cases we offer fee-based advocacy services to assist the family in obtaining all possible financial and social services available. In many cases significant resources may be obtained through various state, local and federal sources.
Navigating through the requirements of these institutions is extremely tedious. We have 2 decades experience and unmatched ability in assisting with local, state, federal and private institutions. Avalon Referral Services has a very high level of competence advocating on behalf of others within these institutions.
Senior Care is Senior Thriving, There are many options:
Independent living is for individuals requiring minimal assistance and are frequently know as "retirement or senior communities". Truly in your own home or apartment, but with access to appealing amenities:
- Travel and recreational activities
- Meal Plans
- Housekeeping and Transportation.
- Typically you will not see supervised medical care.
Assisted Living
Assisted Living is great option for persons requiring daily assistance. Assisted Living residences offer some, or all of these services:
- Social Activities, Transportation & 24 -hour support, Meal Plans and snacks.
- Grooming, Bathing and Wardrobe assistance
- Medication and limited Medical Management.
- Housekeeping and Laundry assistance.
Board and Care, Also called Care Homes
Board and Care homes are usually homes in residential neighborhoods, offering many similar services as larger facilities in a home-like setting. Often caregivers live on property. At these homes expect to see:
- Meals, Housekeeping and laundry
- 24 hour support & Transportation
- Medication oversight, Bathing & Dressing.
- Some Recreational Activities
- Occasionally Palliative and Hospice Care
Skilled Nursing Facilities
These facilities are staffed with licensed nurses and medical assistants who are available around the clock. A resident here is typically a senior requiring significant medical care and appropriate supervision.
- Meals, laundry, 24 / 7 support
- Medication management through a Medical Director
- Few Recreational activities
- Palliative and Hospice Care
In Home Care * May require our Advocacy Services
Occasionally the most appropriate living arrangement is right in the family home. This requires the care, cooperation and understanding of the family. Often, the individual is not a senior, but someone with impaired abilities. Our Advocacy Services can help the family navigate the complex local, state and federal requirements to qualify for funds that may allow care to be provided in your own home.
